Approach:

If you have time to learn it properly, see Eric Strong’s YouTube series:

1. Dog’s balls

·     Normal vs abnormal

·     Obvious pathology

2. Rate

Number of big squares:

1.   300

2.   150

3.   100

4.   75

5.   60

6.   48

Multiplication:

·     Rhythm strip = 10 seconds

·     HR = n(QRS) x 6

3. Rhythm

Where:

·     Rhythm strip ± Lead V1

·     Horizontal analysis – beat by beat

·     Vertical analysis – line up with other leads

P:

·     Regular vs irregular?

·     Number of morphologies?

·     Each followed by a QRS?
(DO NOT SAY: is each QRS preceded by P wave – N Strathmore)

QRS:

·     Regular vs irregular?

·     Broad vs narrow?

·     Number of morphologies?

4. Axis

I

aVF

Axis

Normal

If ↑2: Normal

If ↓2: Left

Right

Extreme

5. Waveforms

What:

·     P (wide? tall?)

·     PR (long? short?)

·     Q (abnormal?)

·     QRS (width? amplitude? R-wave progression?)

·     ST (up? down?)

·     T (inverted?)

·     QT (long?)

Where:

·     Inferior: 2, 3, aVF

·     Anterior: V1-4

·     Lateral: 1, aVL, V6
